Why does recovery time vary from three to six months for breast augmentation?
The combination of breast implant choices that you make will affect how long your breast augmentation recovery takes. You will make choices of the type, size and shape of your breast implants.
Each type of breast augmentation is associated with preferred incision sites. Procedures that involve more or larger surgical incisions, such as sub-pectoral placement of your implants, silicone implants or significantly large implants require longer healing times.
Saline implants that can be inserted flat and filled after placement are inserted through a smaller incision. Sub-glandular implant placement does not require a second incision into the muscle, so will not require as long a breast augmentation recovery period.
The recovery period also includes the time it takes for your breast implant to settle into its natural place and therefore achieve the natural breast shape you desire.
What is capsular contracture?
Capsular contracture is the tightening of scar tissue that forms around the implant. When the scar tissue develops too tightly, it forms a capsule that shrinks and changes the contour of the skin over your breast implant. There are four grades of capsular contracture:
- Baker grade 1: soft breast, capsule not contracted
- Baker grade II: slightly firm breast that looks normal, mild contracture
- Baker grade III: breast feels firm, appears distorted
- Baker grade IV: breast feels firm, appears distorted and causes pain

Breast Augmentation Incision Options

Incision Options for Breast Augmentation
- Inframammary Incision – Made in the crease below your breast where it meets your chest, allowing the scar to be virtually unseen and easily hidden by clothing.
- Periareolar Incision – Made around the areolaso that scarring blends in with the darker skin. Drawbacks include residual numbness of the nipple and a higher risk of complications with regard to breastfeeding.
- Transaxillary Incision – Made in the armpit so the scarring is very inconspicuous. This incision type is the most difficult for breast implant placement.

Can I still get a mammogram with breast implants?

Breast implants obscure about 19% of the breast tissue so an ultrasound machine has to be used to see all of the breast tissue. Breast augmentation has actually been shown to help in early detection of breast cancer because the extra views have to be taken.

How long do implants last?

Breast implants can last a lifetime if there aren’t any issues with the implant and the implant stays intact. Some women will have them replaced for cosmetic reasons. They might either want to go bigger, smaller, or just have them taken out. The rupture rate is about 1-2% per year. Saline will just be absorbed by the body and deflate over a period of 3-4 days. If you have silicone, you may not know you have a rupture.

What is the difference between textured and smooth implants? Which is best?

Smooth is better because they are softer and feel better. Textured breast implants are firmer. Some doctors believe that textured implants decrease the risk of capsular contracture but studies have not been able to prove this.

How do you determine what size the implant should be?

In order to determine the best size of a breast implant on our patients, we do Measurements of the chest wall, bra size pre-operatively, height, weight, and the patients goals. Some women do not have enough breast tissue to stretch and accommodate a large implant. It is extremely important to discuss goals and expectations with a doctor before considering breast augmentation.

What types of anesthesia do you use for breast augmentation?

The type of anesthesia we use for breast augmentation at the Plastic Surgery Institute of Southeast Texas is TIVA – Local Anesthesia and IV sedation. This means that the patient is asleep and doesn’t feel anything. The patient is breathing on their own and only given medication through an IV.
A general anesthesia at the hospital, the patient will be intubated and given gases. With TIVA, the patient can recover a lot faster and can go home soon after surgery and recover in their own home. Less medication is used with TIVA and it decreases the risk of nausea and vomiting after surgery.

What are the differences in results between silicone gel and saline implants?

Both breast implants are great and neither causes any type of health issues of any sort. Most of the time it really depends on what the patient desires. Saline implants are cheaper, the scar is smaller, and the chances of a capsular contracture are about 0%. Saline implants are more likely to cause rippling over time than silicone implants. Saline implants are adjustable so any asymmetries can be corrected with this.
Silicone implants are more expensive, the scar is longer, and the chance of capsular contracture is about 10%. They are less likely to ripple and feel more like normal breast tissue. If a patient doesn’t have a lot of breast tissue, it may be recommended to go with silicone because it will look better than saline.

What are the risks with breast augmentation surgery?

Bleeding and infection can be risks after breast augmentation. It is very rare but it can happen. The breast can bleed internally and the breast can get very large over a couple hours. In this case, the patient will have to be taken back into surgery and have the breast irrigated and cleaned out thoroughly. Also, an infection can happen but this too is very rare. This is a risk with any type of surgery. In both instances, the implant will have to be removed and a drain put in. The patient will have to wait 6 weeks and then the breast can be re-augmented. Another risk with breast augmentation would be sensation loss but this usually happens if the patients go too big on an implant and the nerves get stretched.

Do breasts really make noise after breast augmentation?

No, breasts do not make noises after breast augmentation, this is a myth. The breast implants are underneath the breast tissue and sometimes underneath the muscle as well. There is a lot of coverage over the implant. You may be able to feel the implant on the lateral side of the breasts because the muscle doesn’t cover that part of the implant.
